Boulder General Dentistry | Dr. Birnbach, D.M.D.

We will start out by sending you a welcome pack; in it you will find a medical history form. Please complete the form at home and be sure to include any medication you are taking. Mail, fax, or bring the form with you to your appointment. When you arrive, we'll spend some time with you before Dr. Birnbach looks at your teeth, so we can discuss your concerns and desires. Then Dr. Birnbach will carefully examine your mouth, teeth and gums and address any other conditions you are concerned about. Dr. Birnbach may need to take an x-ray scan of your jaw/mouth/teeth. We allow at least one hour for an initial exam. Once we know exactly what is needed, we will book an appointment for any treatment that is necessary.
